Position Summary: Develop and maintain quality referral relationships with social service agencies and provide follow-up and case management services for individuals referred by Outreach and Health Ministry staff.

General Purpose:  Assist program participants, many of whom are experiencing homelessness or are at risk of homelessness and are medically vulnerable, achieve housing and health stability.

Position Responsibilities:

  • Develop and maintain quality relationships with referral resources in or near neighborhoods served by the Outreach and Health Ministry. Resources should cover key needs presented by OHM participants, including but not limited to spiritual care, emergency and transitional housing, health, mental health, and addiction services.
  • Accurate and timely data entry in external and internal data bases, i.e. Electronic Health Records (EHR) and Efforts to Outcomes (ETO).
  • Develop and maintain relationships with OHM participants by accompanying Outreach staff on Street Medicine Outreach and the Health Outreach Bus. Shifts will vary from evenings/ days/ weekends.
  • Provide case management services to OHM participants, by assisting them with day-time follow-up on referrals made by Outreach staff.
  • Maintain case load and accompanying files, ensure that all documentation is in compliance with government and agency standards. Maintain and submit required documentation in an accurate and timely manner.
  • Participate in periodic community needs assessments, and maintain a familiarity with the issues related to the target population, including current literature, available resources, and street dynamics.
  • Build supportive relationships with short- and long-term volunteers in order to provide volunteers with a high-quality experience and to develop strong volunteer support for The Night Ministry.
  • Drive the Outreach Van as scheduled.
  • Be available as a listening ear and resource to people the Street Medicine Program visits; establish relationships and build rapport in a non-judgmental way; employ motivational interviewing and harm reduction methods to engage visitors in services available; offer referrals for social services
  • Must be trauma competent, LGBTQQIA (Lesbian, Gay, Bisexual, Transgender, Queer, Questioning, Intersex, Asexual) competent, and street culture competent, and able to work with diverse populations.
  • Keep up-to-date with the issues related to the target population, including current literature, available resources, and street dynamics.
  • Able to work weekends and weekdays with the Bus Team and Street Medicine Outreach Team
  • Attend staff, agency, and inter-agency meetings as required; assist in representing the program and agency to congregations and other groups; participate in the agency's public relations and fund raising activities as assigned.
  • Must meet all governmental, agency and grantors' qualifications for the position
  • Must exhibit the ability and willingness to work individually, as well as part of a team
  • Other duties as assigned.


Essential Skills and experience:

  • Bachelor's Degree in social service or related field required from an accredited college or university.
  • Two to five years' experience in outreach, health, or case management
  • Detail-oriented self-starter with demonstrated interpersonal skills and a clear sense of professional boundaries.
  • Must have current driver's license with a good driving record and access to car. Willingness and ability to travel locally.
  • Must be comfortable working with diverse faith traditions in nonjudgmental manner.
  • Ability to respond calmly to emergencies and to quickly assess and respond in a safe and appropriate manner in accordance with Night Ministry policies.
  • Comfortable working with homeless people, sex workers (all genders), youth, people from many ethnic and racial communities, and people who are gay, lesbian, bisexual, or transgender.
  • Excellent interpersonal skills.
  • Collaborate to locate, create and maintain relationships with the homeless through outreach. This includes participation in van based outreach, on foot outreach in selected areas, staffing of daytime or evening programming, where the homeless are likely to congregate.
  • Comfort with developing and implementing new venues and methods of outreach.
  • Familiarity with referral sources
  • Strong verbal and writing abilities
  • Excellent organization skills and attention to detail.


Reporting to this position:  No direct reports

This position will be a part-time positon through December 31, 2017 with the potential to transition to full-time January 1, 2018.  Transition to full-time is dependent upon funding.

  • Physical Demands: Physically able to walk and/or stand on the street, walking under viaducts, through fields and woods, in all seasons, for as long as six consecutive hours.
  • While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle or feel objects, tools or controls; climb stairs; crouch, or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move 50 pounds and drive an automobile. The employee must be able to stand outdoors for as long as six hours or more, exposed to all varieties of Chicago weather conditions. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Work environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time. The noise level in the work environment is usually moderate, but may vary at the different locations required in this job.
